Asuhan Kebidanan Continuity of Care (COC) pada Ny. YF dari Masa Hamil Sampai dengan Masa Interval

Abstract
Angka Kematian Ibu (AKI) dan Angka Kematian Bayi (AKB) di Indonesia masih tinggi, terutama disebabkan oleh perdarahan, infeksi, hipertensi dalam kehamilan, serta komplikasi persalinan dan nifas. Salah satu upaya untuk menurunkan AKI dan AKB adalah dengan menerapkan Continuity of Care (COC), yaitu pelayanan kebidanan yang berkesinambungan mulai dari masa kehamilan, persalinan, nifas, neonatus, hingga keluarga berencana. Tujuan laporan ini adalah untuk memberikan asuhan kebidanan berkelanjutan pada Ny. YF mulai dari masa kehamilan sampai masa interval. Penulisan menggunakan metode studi kasus di PMB Enik Murdyati, Kecamatan Grogol, Kabupaten Kediri dari November 2024 hingga Mei 2025 dengan pendekatan manajemen kebidanan berbasis SOAP. Pengumpulan data dilakukan melalui wawancara, observasi, dokumentasi, serta menerapkan etika asuhan. Pada masa kehamilan Ny. YF mulai dilakukan asuhan pada usia kehamilan 31-32 minggu. Persalinan spontan dilakukan di PMB Enik Murdyati karena ibu datang dengan pembukaan lengkap. Asuhan neonatus diberikan dengan perawatan metode kanguru dikarenakan bayi BBLR. Masa nifas berlangsung normal, dan ibu memilih metode kontrasepsi Implan. Asuhan kebidanan berkelanjutan efektif dalam mendeteksi dini komplikasi dan memastikan ibu menjalani masa kehamilan hingga KB dengan aman dan lancar.
